RGN required for Respiratory Integrated Care team Co Donegal
This is a post within the Respiratory Integrated Care team who are currently undertaking a Slaintecare Virtual Ward Project.
The post would require cover Monday – Friday from 8am to 4pm with no weekends or bank holidays included.
It entails working within the Multidisciplinary team in a community environment.
The staff member would be based in Letterkenny but would need to have the ability to travel within Donegal as the RIC service is operational in all 4 network locations throughout Donegal.
Preferable Requirements if possible
* 2 years post-registration general medical nursing experience.
* Experience in respiratory chronic disease nursing care is desirable.
The role of the staff nurse involves working within the respiratory integrated care team (RICT)
The responsibilities within the team include:
* Assessing, planning, implementing, and evaluating nursing and respiratory care.
* Monitoring and managing patients' symptoms.
* Be involved in the analysis of remote monitoring of clinical data to identify potential issues and provide early intervention.
* As part of the RICT, provide individualised patient-centred care virtually and face-to-face when required.
* Carry out home visits as necessary to assess the patient's needs, monitor progress, and liaise with the RIC team.
* Educate patients and their families about their respiratory condition, treatment plans, medication management, and lifestyle changes.
* Provide open-access telephone support to patients and families.
* Maintain links and communicate effectively with community services through the multidisciplinary team, patient, and family to ensure continuity of patient care.
* Refer to other services as necessary and signpost to other support services.
* Discuss discharge planning and ongoing care.
