Family Multi-Disciplinary Team (MDT) Manager
Location: Dublin
Contract: Full time, Permanent
The primary purpose of this role is to co-ordinate, plan and deliver a Multi-Disciplinary Team in the Dublin region to work with families experiencing homelessness or who have recently exited homelessness.
What you will do:
1. Develop relationships and structures with relevant stakeholders to develop and eventually deliver an MDT service for families with complex needs.
2. Develop links, networks and establish working MOU’s to deliver clinical aspects of the recommended MDT service and inform the prioritisation of any emerging developments where potential service gaps are identified.
3. As required appraise and develop service options / proposals for vulnerable families.
4. To work closely with the heads of service, advocacy and practice development teams to develop frameworks and practices for delivering a fully realised MDT service.
5. To project manage the delivery of the new MDT service reporting and updating the heads of service regularly on progress, challenges and opportunities.
6. Develop and present a costed business case for provision of services as per feasibility study recommendations.
7. Maintain appropriate, up-to-date written and electronic records and activity data in accordance with professional service standard and provide reports and data as required.
8. Efficiently and effectively manage and monitor budgets / expenditure and resources as may be assigned, and to prepare financial management reports as may be required.
9. Be responsible for the provision of data/statistics as requested.
10. Develop and promote multidisciplinary, interagency, intersectoral links with relevant statutory, voluntary and private service providers as appropriate.
11. Work in close liaison and co-operation with other teams and departments within Focus Ireland.
12. Attend Team, Section, Divisional, Agency and external meetings, including handovers, and training as required.
13. To implement existing policies and procedures and to input into the development of new ones.
14. Engage in supervision sessions on a regular basis with the head of services.
15. Undertake such other duties as might be reasonably assigned from time to time in consultation with the Head of Services.
16. To be vigilant to any Health, Safety and Welfare risks in the workplace and bring any concerns to the attention of your line manager or Health & Safety Representative.
17. Work within the framework of the overall objectives, policies and procedures of Focus Ireland.
What you will need:
18. A recognised third level qualification (minimum Level 8) in social care/work, nursing, public health, psychology, addiction studies or a related field.
19. At least two years’ management experience working with homeless people, substance users, people experiencing poor mental health and/or other socially marginalized groups.
20. Experience of working with vulnerable families with complex needs.
21. Experience of planning, report writing and meeting deadlines.
22. Experience of sourcing funding for new projects.
23. Experience of working in a multi-disciplinary team and liaising with other agencies, including community, voluntary & statutory.
24. Full Driver’s License with access to a car.
25. Experience of working within a care & case management model or demonstrable equivalent.
26. In-depth understanding of key principles of care & case management approach.
27. Excellent communication & interpersonal skills.
28. Excellent influencing skills.
29. In depth knowledge and understanding of homelessness and issues affecting families with complex health, mental health and addiction needs amongst others.
30. Excellent project management skills and ability to implement programmes, relevant to key strategies e.g. Housing For All, Slaintecare, National Drugs Strategy and Housing First.
