As Person Support Coordinator you will work professionally, sensitively, and through a relationship approach with individuals in a HIQA designated home,ensuring a safe, homely environment where each person is holistically supported. You will work directly with individuals as part of their circle of support and be responsible for the day-to-day oversight of service and the household. Reporting to the Person in Charge, your focus is on assuring a quality service enabling each individual to live a fulfilling life based on their will, preference, and choice, valued social roles, community participation, and community presence. As Person Support Coordinator, you are a vital member of the individuals circle of support, working with them, and ensuring day-to-day communication with their team, other stakeholders, and the Person-in-Charge. You will actively support the team to ensure that the individuals support focuses on their rights, choice, will, and preferences. You will be expected to work effectively with the team ensuring that all team members are providing supports in line with Social Role Valorisation (SRV), United Nations Convention on the Rights of People with Disabilities (UNCRPD) and St. Margarets Core Values. You will be responsible for supporting each individual through St. Margarets framework of Supported Self-Directed Living (SSDL). As a member of the individuals circle of support, you will work with them to support them to gain the experience and develop the skills that will support and enable them to make choices and develop connections as they live a life of their choosing at home and in their community while continuing to flexibly respond to their changing lifestyle choices throughout their life. Key Roles of Person Support Coordinator Minimum QQI Level 5 in Health and Social Care or equivalent Allied Health discipline 3 years experience in a similar role Coordinator & Team Player As Person Support Coordinator, you are a vital member of the individuals circle of support, working with the individuals, their team, and external stakeholders. You will actively support the individuals goals through collaboration, offering ideas and solutions, and maintaining a positive attitude. Lifestyle Enabler As Person Support Coordinator, you have an important role to play in supporting individuals to achieve the things that matter to them and exploring opportunities for expanding their horizons and interests. You shall build a respectful and supportive relationship that honours their story, values, and preferences, In the individuals home, you will manage the day-to-day operations of the household, ensuring that all household and administrative systems are maintained efficiently. This includes organising team meetings, overseeing the upkeep of documentation, and coordinating schedules to ensure comprehensive support. Active Supporter You will work closely with individuals and their team to support individuals in achieving their personal goals, promoting independence, and ensuring their daily supports are in place and being are met. Your role includes empowering individuals, facilitating their participation in the community, and encouraging new opportunities. Coordinator Safe Practitioner The safety and well-being of individuals are at the forefront of your responsibilities. You will ensure a safe environment within the home by adhering to relevant policies, regulations, and best practices, while balancing support for individuals autonomy. Recorder & Reporter As Person Support Coordinator, accurate documentation and communication are essential. You will maintain comprehensive records and ensure relevant information is shared appropriately with team members and the Person in Charge.
