Confined Competition - Internal Applicants Only Post Title: GP Liaison Nurse – Emergency Department Post Status: PermanentDepartment: Emergency Department Location: Beaumont Hospital, Dublin 9Reports to: Directorate Nurse ManagerSalary: Appointment will be made on CNM 2 Pay scale (€60,854 - €76,897) at a point in line with Government pay policy. Hours of work: Full Time, 37.5 Hours Per Week (1 WTE)Closing Date: Friday 11th Apri 2025 @ 12noon Interviews will be held on Wednesday 16th April 2025 Please note the hospital reserves the right to close the competition early should a substantial number of applications be received. Position Summary:To ensure a good quality integrated hospital/community service to patients presenting at the Emergency Department.To maintain good communication between General Practitioners, Community Services, Emergency Department and patients. To monitor and process OPD referrals from the Emergency Department.To assist the Emergency Department Consultants in their clinical appraisal of patient discharges.
Responsibilities
Principal Duties and Responsibilities: 1. Case Management1.1 Accept referrals to the Emergency Department from General Practitioners/Community Nurses who may alert the Shift lead/Triage nurse of the impending arrival and state of condition of a particular patient.1.2 Respond to the requests of General Practitioners/Community Nurses for reports regarding their patients.1.3 Ensure that the general practitioner/community services are informed of the outcome of a patient’s assessment in the Emergency Department.1.4 Notify the General Practitioner should a patient of his/her be brought in dead or die in the department.1.5 Act as a vital link between the Emergency Services and the Community Care Services as per agreed policies.1.6 Ensure that information concerning patients required by General Practitioners/Community Nurses to be given correctly, methodically and professionally.1.7 Liaise with all staff within the Department, Medical, Nursing, Clinical Services and Ancillary to ensure that at all times information on patients is correct and up to date.1.8 Communicate with the Admissions Office, Wards and Clinical Services when necessary.1.9 Liaise with other relevant community/voluntary agencies as appropriate.2. Case Management2.1 Implement a systematic method for the assessment of individual patient's needs and ensure proper documentation of same.2.2 Estimate workload, prioritise and plan same. 3. Staffing and Manpower Planning3.1 Support nursing staff and other support staff in the relationships with the Community Services.4. Personnel4.1 Assist the Clinical Nurse Manager 3 (CNM) and/or Directorate Nurse Manager (DNM) in promoting, nurturing and maintaining a high level of staff morale hence promoting team spirit and job satisfaction among nursing and other staff within the Department.4.2 Promote and develop inter hospital relationships and community services pathways.4.3 Maintain good working relationships with all Medical, Nursing, Clinical Services and other Personnel.4.4 Participate in the development of an orientation programme for staff.4.5 Assist the CNM with the investigation of incidents, accidents and complaints involving patients and staff. Report these verbally and in writing to the Directorate Nurse Manager. 5. Education/Staff Development5.1 Assess and identify training and development needs for nursing staff in relation to Community Liaison Services and set objectives for such learning; recommend these to the CNM2 and contribute to the development of training programmes where appropriate.5.2 Assess and identify training and development needs for nursing staff in relation to administration requirements for patients who die in the Emergency Department and set objectives for such learning; recommend these to the CNM and contribute to the development of training programmes where appropriate.5.3 Assist the CNM3/DNM with the identification of teaching/learning opportunities within the department.5.4 Assist the CNM 3/DNM by participating in Emergency Department Induction Training of all new nursing staff ensuring that staff nurses receive sufficient theoretical and practical training to make them safe practitioners.5.5 Assist the CNM 3/DNM in ensuring that nurse training requirements are met.5.6 At all times act as an effective role model by demonstrating skilled nursing care in the clinical situation for staff and student nurses. 6. Clinical Nursing Practice6.1 Use a framework and model of nursing to assess individual patient needs consistent with hospital nursing policy.6.2 Ensure that appropriate nursing care is planned for relevant individual patients based on need assessment and that each individual patient receives prescribed treatment and nursing care, encompassing medical instructions and in conjunctions with established procedures and standards of care.6.3 Ensure that verbal and written clinical reporting systems are accurate and adequate.6.4 Assists the CNM to direct, supervise and mentor the nursing staff in the management of nursing care ie providing professional and specialist advice on the provision of nursing care and demonstrated practical nursing skills.6.5 Ensures that patients and relatives are appropriately informed of follow up treatment and that appointments are co-ordinated.6.7 Participate in research when necessary.6.8 Attend and participate in multidisciplinary meetings as required.6.9 Effective skills in the open disclosure process. 7. Administrative Role7.1 Follow up on issues which arise out of hours and at weekends.7.2 Keeps up to date on all Hospital policies particularly those relevant to the Emergency Department.7.3 Maintain records of all dealings with the General Practitioners/Community services.7.4 Maintain accurate records of all patient caseloads that have been retrospectively assessed by the Emergency Department Consultants. 8. Self-DevelopmentThe post holder is expected to:-8.1 Maintain, update and develop knowledge on relevant professional development on all Speciality current trends by attending a number of study days each year.8.2 Maintain a personal record of professional development.8.3 Accept appraisal each year from CNM 3/DNM.8.4 Discuss, present performance and future self-development needs with the Divisional Manager and CNM 3/DNM, using job performance review in accordance withDepartment policy. Qualifications
Selection Criteria: Selection criteria outline the qualifications, skills, knowledge and/or experience that the successful candidate would need to demonstrate for successful discharge of the responsibilities of the post. Statutory Registration, Professional Qualifications, Experience, etc(a) Eligible applicants will be those who on the closing date for the competition:(i) Are registered in the relevant division of the Register of Nurses & Midwivesmaintained by the Nursing and Midwifery Board of Ireland (NMBI) (BordAltranais agus Cnáimhseachais na hÉireann) or entitled to be so registered.(ii) Have at least 5 years post registration experience (or an aggregrate of 5 yearsfulltime post registration experience) of which 2 years must be in the specialityor related area.(iii) Have the clinical, managerial and administrative capacity to properly dischargethe functions of the role.(iv) Candidates must demonstrate evidence of continuous professional development.(b) Candidates must possess the requisite knowledge and ability including a high standard ofsuitability and clinical, managerial and administrative capacity to properly discharge thefunctions of the role.2. Annual registration(i) On appointment, practitioners must maintain live annual registration on the relevantdivision of the Register of Nurses and Midwives maintained by the Nursing andMidwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann).(ii) Confirm annual registration with NMBI to the HSE by way of the annual PatientSafety Assurance Certificate (PSAC).3.
