Role Overview
The Programme Facilitator role supports individuals with complex needs to achieve and maintain valued roles in their community through learning, social and recreational opportunities.
Duties and Responsibilities
1. Act as Key Worker for Service Users, identifying goals and supports required and maintaining records in order to facilitate the Person-Centred Planning process.
2. Develop and deliver Person-centred programmes and maintain records of same in order to meet the needs of Service Users that have been identified through the Person-Centred Planning process.
3. Support individuals and groups in accessing their community by facilitating activities, programmes and any other reasonable duties as deemed appropriate by the Programmes Supervisor or Community Services Manager in order to meet identified needs.
Requirements
* Minimum QQI Level 5 in Social Care or equivalent
* A third level graduate, Health & Social Care or equivalent is desirable.
* Experience supporting adults with varying disabilities.
