About the Employer Charlene Cooke Healthcare Recruitment is thrilled to partner with a Primary Care centre in West Clare to recruit a dedicated and enthusiastic Staff Grade Occupational Therapist. This is an exciting opportunity for a newly qualified or experiencedOccupational Therapist. The model of service is mainly home based with interventions provided including falls prevention, cognitive rehabilitation, community skills training and self-care reablement. The Occupational therapist will also provide clinic-based services such as lifestyle redesign groups, assistive technology training, complex seating assessments and upper limb rehabilitation. The primary focus is on helping the client to achieve their maximum potential in the areas of functional participation, health promotion and occupational wellness. Details of Service: The person appointed to this post will work as part of multidisciplinary teams delivering a coordinated approach to client care either in Adult services. The Occupational Therapy Department within Community Healthcare Services in West Clare provide high quality, holistic and client centred care at the right time and in the right place to those living in the community. Purpose of the post: To work in conjunction with Occupational therapy and multidisciplinary team members in coordinating and developing the service to meet the needs of the service user group and the objectives of the organization. The Occupational Therapist will be responsible for the provision of a high-quality Occupational Therapy service. Key Responsibilities: Responsible for assessing, planning, implementing, and reviewing treatment programs. Manage caseload and collaborate with service users, families, and staff in treatment planning. Plan service user discharge or transition appropriately. Document assessments, treatment plans, and progress notes according to standards. Communicate assessment results and treatment recommendations to the team. Participate in team meetings, clinics, and case conferences. Perform duties timely, maintain quality standards, and participate in audits. Seek advice when needed and adhere to professional standards and ethics. Operate within Occupational Therapy practice guideline Requirements: Be registered on the Occupational Therapists Register maintained by the Occupational Therapists Registration Board at CORU. Primary Care/ Community services experience (undergraduate placements considered). A team player with excellent communication skills and a passion for working with children. A full drivers license and access to your own vehicle. Current residency in Ireland with the legal right to work without restrictions. Willingness to complete Garda Vetting.
