Clinical Nurse Specialist Cardiovascular Disease - Integrated Care

Location of Post: Ballinasloe Ambulatory Care Hub, HSE West and North West, 1st Floor, Right Side, St Brigids, Ballinasloe, Co Galway.

Role Responsibilities
The CNSp will deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse/Midwife Specialist Post:
* Clinical Focus
* Education and Training
* Audit and Research
* Consultant
Clinical Focus: The CNSp will provide a specialist nursing service for patients with cardiovascular disease, incorporating evidence-based knowledge and specialist assessment techniques.
Education & Training: The CNSp will maintain clinical competence, provide education to patients and develop training programs for MDT colleagues.
Audit & Research: The CNSp will collect and maintain relevant data, conduct audits, and contribute to nursing research in cardiology nursing care.
Consultant: The CNSp will provide leadership in clinical practice and develop collaborative working relationships with local cardiology teams.
Eligibility Criteria
Qualifications and/or experience: Applicants must have:
* Statutory Registration as a nurse/midwife on the active Register of Nurses and Midwives.
* A minimum of 1 years’ post-registration experience in the General division.
* A minimum of 1 years’ experience in the specialist area of Cardiovascular Care.
* A post-registration programme of study relevant to Cardiovascular Care.
Other Requirements specific to the Post: Access to own transport is required as a significant portion of the work will be off-site.
