Due to this expansion of our service provision to a Dublin Hospital, an exciting opportunity has arisen for a pharmacist with relevant hospital experience or an interest in hospital pharmacy, to provide a clinical pharmacist service to the hospital. This role will be on a part-time basis e.g. 3 half days (mornings).
The role will include:
1. Medicines reconciliation – ensuring all medications on admission have been charted correctly onto patient Kardex.
2. Communication as appropriate to medical and nursing teams.
3. Following up on previous communications ensuring closure of loop on any outstanding issues.
4. Clinical appropriateness checking (dosing, major interactions, following local guidelines etc.).
5. Liaising with nursing and medical teams to provide the best clinical care.
6. Liaising with community pharmacy management re stock issues.
7. Participation in MDT meetings (quarterly).
8. Oversee stock management in partnership with pharmacy technician.
Team working and excellent communication skills are essential for this role as well as the ability to work on own initiative.
