1840 West Dr, Vista, CA 92083, USA Req #8407Thursday, April 3, 2025Position Summary:The Site Medical Director provides medical care for San Diego PACE enrolled participants across a variety of settings, including but not limited to the center's clinic, participants’ homes, the nursing home, assisted living facilities and personal care boarding homes. Direct emergency department and hospital inpatient care is not required but close coordination of care with hospital-based providers is essential. The Site Medical Director is also responsible for oversight of day-to-day medical operations at the center and provides supervision of physicians, physician assistants and nurse practitioners. The Site Medical Director is responsible for oversight of the department budget, quality initiatives, and utilization and resource management. Physical attendance at a San Diego PACE facility and/or a San Diego PACE participant home is essential to performing the job duties.Essential Functions of the Job:Primary Care - 50%Provide evidence-based and cost-effective medical care to enrolled participants.Evaluate, diagnose and manage their medical conditions, including but not limited to taking of a medical history, performance of physical examination, ordering and interpretation of diagnostic tests, formulation of an assessment of condition and a treatment plan, and plan of care, writing prescriptions, ongoing follow up of same, and oversight of care and medical information from specialists and hospital teams and other providers such as nursing homes and hospice agencies.Document medical care in the participant’s medical record, in compliance with documentation policies.In collaboration with the PACE Inter-Disciplinary Team, provide direct oversight of all the care and services ordered or approved for your participants.On a rotational basis with other Medical Directors provide after-hours back-up phone on-call every few weeks. All calls that require Primary Care Provider’s (PCP’s) attention are directed by the call service to the after-hours on-call PCP for advice or orders. As the Medical Director on-call, you will provide back-up for the PCP (phone support).Participate within the interdisciplinary team in the formulation of Plans of Care for participants, as well as in other interdisciplinary team settings that further the care of participants.Present input into, initiate and/or facilitate new medical programs and processes in support of strategic goals.Administrative & Supervision – 50%Assists with the preparation of the primary care budget, and explains monthly budget variances.Provides reports on utilization areas: Inpatient, ER, Outpatient, Nursing home, Pharmacy and areas where there is a significant budget variance.Participates in the quality initiatives in conjunction with the Quality Department.Responds to center grievances with respect to primary care complaints.Champions at least one primary care strategic initiative.Coordinates the regular primary care team meetings held at the center and may lead local primary care meetings.Responsible for the recruitment, hiring and termination of primary care staff under the direction of the Associate Chief Medical Officer.Oversee the provision of care and oversight provided by all Center physicians, physician assistants and nurse practitioners.Ensure that providers are practicing in an evidence-based and cost-effective manner, and as part of the Inter-Disciplinary Team.Ongoing evaluation and training of site physicians, physician assistants and nurse practitioners.Supports Best Practices and appropriate utilization initiatives including HCC Coding and Risk Adjustment, Medication Management, and oversight of testing, treatment and referrals.Oversight of clinical, operational and regulatory quality standards.Involvement with the electronic medical record (EPIC) as a Subject Matter Expert (SME) for the site’s clinic and providing the Associate Chief Medical Officer with input around needed ongoing EMR enhancements.Effectively supervises and manages direct reports and provides, by example and leadership, motivation and direction to employees of the organization.Helps set the tone of the department to ensure morale, teamwork, and that the positive employment culture of the organization is maintained.Regularly schedules leadership/management, department, and staff 1:1 meetings to provide leadership, development, and a forum for communication to ensure coordination and collaboration in meeting organization and individual goals.Additional Duties and Responsibilities:Back-up to other Site Medical Directors as needed.As assigned by the Associate Chief Medical Officer.Job Requirements:Education Required:Doctor of Medicine or Osteopathy degree from an accredited school of Medicine.Certifications Required:Current CA MD/DO.DEA License with full prescriptive authority.BLS certification.Board certified or eligible in primary care specialty.Experience Required:A minimum of one year working with the frail or elderly.Technical Knowledge and Skills Required:Proficient with Electronic Medical Records, ideally EPIC.Working Conditions and Physical Requirements:Center-based office practice Monday to Friday 8am to 5pm.Requires repeated sitting, standing, walking, bending.Universal Requirements:Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs.
