Cpl Healthcare in partnership with our client, a non-profit organisation based in Dublin, Ireland, provides services for people who are homeless and those at risk of homelessness in Ireland.
The purpose of this role is to provide visiting support to households, enabling a sustainable exit from homelessness and helping them settle into their homes and communities. Through this support, customers are assisted in building the skills and confidence needed to manage their homes and address tenancy challenges. This role is part of the Visiting Tenancy Sustainment Service (VTSS) and the Settlement Service (SLI), a partnership between Dublin City Council and our client, to support households in sustaining their tenancies.
What you will do:
* Work within the framework of the overall objectives of the organisation, in accordance with the Service Level Agreement between Dublin City Council and our client.
* Ensure adherence to Dublin City Council Generic Housing and Health and Safety Policies.
* Work within the agreed caseload.
* Engage customers effectively in keeping with the covenants and responsibilities set out in Dublin City Council Tenancy Agreements, Policies, and Systems for rent collection, maintenance, and estate management.
* Contribute to the formulation of customer’s needs assessment and support plan utilizing the PASS system and keep up-to-date case work records.
* Through a model of visiting support, provide one-to-one engagement to ensure that the Customer is supported in making informed decisions relevant and in compliance with the challenges and responsibilities they face as a household and under tenancy to the Local Authority.
* Contribute and participate in conjunction with the Manager/Team Leader in ensuring a package of support is designed and implemented by the Case Manager which promotes the welfare of the household.
* Work as part of a multi-disciplinary/multi-agency service.
* Use a flexible approach in engaging households with their individual needs, considering dynamics relevant to their needs and behaviours.
* In conjunction with the management team, develop and maintain links with the Garda, Probation, Health Service Executive, Medical Services, Community-based services, and Local Authorities.
* Ensure effective communication, referrals, case work, case reviews, and discharge planning where appropriate.
* Contribute to a case conferencing model ensuring effective liaison with external professionals and services and the Customer where appropriate.
* Ensure appropriate liaison with all stakeholders in the household support circle, including as appropriate Social Workers, Schools, G.P.’s, and other clinical service practitioners.
* Accompany and provide support to Customers on appointments to other services e.g. doctors, counsellors, social workers if required.
* Offer flexibility - the service runs Monday to Friday; however, some weekend and evening cover may be required from time to time.
* Attend and contribute to meetings and reviews as required and follow up on decisions made between the management team and Local Authority representatives.
* Implement the policies and procedures of the SLI/VTSS service and Focus Ireland, inclusive of Health and Safety policy now and in any amendments in the future.
* Attend team, sectional, divisional, and any other organisation meetings as required.
* Engage in supervision sessions on a regular basis with the manager.
* Participate in relevant training and development courses as agreed with your line manager.
* Undertake such other duties as might be reasonably assigned from time to time in consultation with the management team.
* Be vigilant to any Health, Safety, and Welfare risks in the workplace and bring any concerns to the attention of your line manager or Health & Safety Representative.
What you will need:
* 2 years of experience working with families and individuals experiencing homelessness.
* BA in Social Care or equivalent qualification.
* Experience of lone working in a social care setting.
* Experience of working and liaising with multiple agencies.
* Computer Literacy and report writing skills.
* Full clean driver’s licence (desirable).
