Job Purpose
The Community Support Worker will assist the Community Services Manager and Team Leader in delivering a quality individual focused service. They will work individually and as part of a team to provide high-quality care and rehabilitative support to adults with Acquired Brain Injury and/or other neurological conditions in the community in a person-centered service.
Responsibilities
* To support service users in carrying out rehabilitation programmes for physical, cognitive, and emotional impairment to maximize quality of life.
* Liaise with families, multidisciplinary teams, and other personnel as directed by the management team to ensure the best quality of service.
* Provide personal assistance in all aspects of personal care and manual handling, ensuring comfort and hygiene of service users and adhering to safe practice and risk assessments at all times.
* Ensure that all interactions with service users are approached with dignity, respect, equality, and incorporating choice.
* Actively participate in educational, recreational, and occupational programmes enabling service users to integrate with their local community and access local resources.
