The Social Care Worker will provide support to individuals with intellectual disabilities, promoting independence, inclusion, and community integration. This role requires a strong commitment to person-centred practice, working collaboratively with individuals, families, and communities to achieve positive outcomes.
Key Responsibilities:
* To assess and support the needs of individuals with intellectual disabilities, developing and implementing individualised care plans.
* To work collaboratively with colleagues, families, and external agencies to ensure effective communication and coordination of support.
* To promote community inclusion and participation, empowering individuals to live independently and make choices about their lives.
* To maintain accurate records and reports, adhering to relevant policies and procedures.
Requirements:
* QQI Level 7 (or higher) qualification in Social Care/Applied Social Studies or equivalent.
* CORU recognised Social Care Qualification.
* Experience working within the area of intellectual disability and those who have behaviours of concern.
* A holistic and person-centred approach to your role, ensuring individuals are supported to live their best lives.
