Are you an RMN or OT with evidence of community and dementia experience? The Intensive Community Support Dementia team is looking for a Mental Health Nurse to join their supportive team based at Forston Clinic, Dorchester!
We are offering a unique opportunity for a fantastic development post from a Band 5 to a Band 6 Clinical Team Leader role. Full training will be given, and you will have the opportunity to expand your nursing skills. As this role covers North and West Dorset and is based in the community, a driver is essential for this role; pool cars are available.
We are a specialist service in which Community Mental Health Teams can refer patients when their behaviors become more challenging. We offer high-quality interventions to help patients in their own environment, whether that be a care/residential home or their own private home.
You will need to be dynamic and highly motivated, with exceptional assessment, risk management, and care formulation skills. Communication skills and clinical decision-making are essential in this role. ICSD is a 'virtual ward,' and senior nursing staff must be able to process information in a variety of ways accurately and quickly.
We are looking for either Full Time or Part Time role with a minimum of 30 hours per week. ICSD operates 7 days a week, from 07:30-20:00. We work on a rota basis covering 07:30 - 15:30 / 12:00 - 20:00, alternating weekends at enhanced pay.
Main duties of the job
You will work with CMHTs, social care, and other agencies to benefit patients and support carers while patients are under our care and signpost for ongoing support. An understanding of MHA, MCA, and safeguarding legislation is essential.
* Undertake assessments, nurse reviews, manage a small group of patients, and triage referrals as they come in.
* As a Clinical Team Leader, you will undertake shift management ensuring the smooth running of the team, using resources effectively.
