We would like to invite applicants for the role of Patient Flow (ADON) in Bartra Healthcare.
Purpose of The Role
To support the multidisciplinary team by scheduling, daily admissions, coordination of bed management, and discharging bed management functions in accordance with Bartra Healthcare Standards of Quality Care. To support Nenagh Rehabilitation Unit and Beaumont Lodge.
Bed Management Responsibilities:
1. Develop rapport with critical stakeholders.
2. Ensure that referral, admissions, and discharge processes are adhered to at all times.
3. Conduct a thorough clinical assessment of each patient referral either on-site or via telephonic assessment as appropriate.
4. Allocate patients to the most appropriate ward/bed available to assure safe standards of nursing care.
5. Monitor bed occupancy and send daily reports to senior management.
General Responsibilities
1. Ensure that the provision of a high standard of care to the residents or patients is consistent with the mission, vision, values, and strategic plan of Bartra Healthcare.
2. Ensure recognition of the resident/patient as an individual, ensuring their rights, needs, and comfort are given priority during the provision of care.
3. Develop relationships with residents/patients, which facilitates their autonomy, informed choice, and evidence-based decision-making.
4. Promote and maintain resident/patients' safety and provide quality care through respecting their privacy, dignity, and rights.
Delegation of Tasks and Duties
1. Ensure that where directing delegation of care, the delegation is appropriate to the definitions and philosophies of nursing. Delegation shall take into account the levels of experience, competence, role, and scope of practice of the staff member taking on the delegated task.
2. Be accountable for roles and activities delegated to junior staff, where appropriate.
3. Work towards a restraint-free environment, with implementation of practices in accordance with the National Policy.
Qualifications
1. A registered General Nurse or Psychiatric Nurse on the Register of Nurses held by Nursing and Midwifery Board of Ireland or be entitled to be so registered.
2. Evidence of continuous professional development.
Desirable:
1. Post Graduate Diploma in Gerontology.
2. Gerontology experience.
3. Experience in Psycho Geriatric Nursing Care/Psychiatry/Adult Disabled.
4. In-depth knowledge of the ageing process.
5. IT Skills.
Desirable:
1. Knowledge of organisation of health services and trends nationally and internationally.
2. Professionalism in the application of theory to daily practices.
3. Critical thinking to be able to recognise resident problems and adapt to changes quickly.
4. Ability to recognise shortfalls or limitations of their competencies.
5. Ability to source both theoretical and technical information.
