Job Title: Intensive Care and Case Management Worker

Cpl Healthcare is partnering with a non-profit organisation based in Dublin, Ireland that provides services for people who are homeless and at risk of homelessness.
Job Purpose
The purpose of this role is to undertake individual work with families and customers who face chronic exclusion due to complex needs, utilising an Intensive Care and Case Management Model.
Key Responsibilities
* Work within the framework of the organisation's overall objectives
* Collaborate with a multi-disciplinary / multi agency service
* Use a flexible approach to engage families with their individual needs, considering dynamics relevant to their needs and behaviours
* Develop and maintain links with Health Service Executive, Tusla, Probation, and other clinical, community and statutory services
* Contribute to a case conferencing model, ensuring effective liaison with external professionals and the family where appropriate
* Provide one to one engagement to support the Family/Customer in making informed decisions
* Implement agreed education programmes, utilising coaching and mentoring interventions
* Contribute to the formulation of a customer's needs assessment and support plan
* Attend and contribute to meetings and reviews as required
Requirements
* 2 years experience working with high need families at high risk utilizing an intensive case management and family support model
* BA in Social Care or Equivalent Qualification
* Experience of working with customers who have co-existing conditions
* Team working experience in social care or clinical disciplines
* Report writing abilities
* Experience of working and liaising with other agencies
* Understanding of issues pertinent to high need families with chronic child welfare issues and homelessness
* Computer skills
* Communication skills
* Knowledge and understanding of children, adolescents and issues affecting parents
* Ability to Plan and Implement Programmes
