Job Description:
The CNM will carry out a range of duties to ensure that we provide a quality, person-centred care environment for all residents. The CNM will work on the floor, assist in supervision of all staff, and be responsible for performance management and ongoing staff training. The CNM will demonstrate managerial and leadership skills and facilitate effective communication with colleagues in the nursing home.
About The Role
The CNM is responsible for the maintenance of a high standard of patient care achieved through professional education and development of all nursing staff, acting as an expert practitioner, managing resources, and facilitating communication. The CNM will manage Nursing and Healthcare Assistants on their team.
Key Responsibilities:
1. Ensure staff allocation adheres to skill and experience considerations.
2. Supervise and develop the nursing team while actively participating in the delivery of care as appropriate.
3. Manage the assigned unit, ensuring safe delivery and coordination of nursing services for patients.
4. Provide clinical and management leadership for Staff Nurse positions.
5. Facilitate effective communication with patients, relatives, visitors, and multi-disciplinary teams.
6. Maintain updated residents’ register for all admissions and transfers.
7. Ensure assessments are completed and entered into care plans for new admissions.
8. Lead family meetings to address residents’ needs.
9. Document end-of-life wishes for all residents.
10. Coordinate Allied Health Professional reviews.
11. Follow up on incidents in files and care plans.
12. Ensure residents are reviewed by the GP every three months.
13. Adhere to Silver Stream guidelines, policies, and procedures.
14. Maintain a safe working environment as per Health and Safety at Work Act.
15. Respect confidential information obtained during professional practice.
Skills and Experience Required:
* Registered in the general division of the Register of Nurses maintained by An Bord Altranais.
* At least three years recent relevant post-registration nursing experience in a nursing home environment, preferably in a senior nursing/team leader role.
* A recognised post-registration nursing course in gerontology is a distinct advantage.
* Proven clinical and professional management ability, clinical audit, and excellent leadership and communication skills.
* Competence in the use of information technology.
* Experience with individuals having additional and/or complex needs.
* Excellent organisational, verbal, and communication skills.
* Self-motivated and able to work as part of a multi-departmental team.
* Reliable, flexible approach to work with high personal standards of performance.
* Excellent problem-solving and decision-making skills.
* Ability to work on own initiative with a responsible and mature approach.
