We are hiring a Clinical Nurse Specialist in Colorectal and Gastro-Intestinal in a Private Hospital in Dublin, Ireland.

Key Responsibilities
* Work with the multidisciplinary team to oversee and coordinate treatment plans, including post-operative care, wound care, ostomy care, and medication management.
* Work with the patient and the MDT to modify the patient's condition management in accordance with established pathways and policies, procedures, protocols, and guidelines (PPPGs).
* Accept referrals from MDT colleagues that are appropriate.
* Organise patient follow-up, treatment plans, and investigations.
* Assess the patient's requirements and offer pertinent support, information, education, guidance, and counselling as needed by communicating with the patient, family, and/or caretaker as needed.
* When necessary, collaborate with Primary and Secondary Care MDT colleagues to deliver services to the patient, family, and/or carer in a seamless manner.
* Support medical and pharmacy professionals by taking part in medication reconciliation while keeping polypharmacy in mind.
Requirements
1. Be registered in the general division of the Register of Nurses maintained by NMBI.
2. Possess a minimum of two years of nursing experience in a specialised field and at least five years of recent, pertinent post-registration nursing experience working full-time or equivalent hours part-time in an acute hospital setting.
3. Possess a major academic award related to the speciality field of colorectal and/or gastrointestinal nursing (or something comparable) at level 8 post-registration Quality and Qualifications Ireland (QQI), National Framework of Qualifications (NFQ), or be willing to obtain one within a year of starting a position.
