Job Purpose
To assist in the delivery of individualized community support to individuals with intellectual, physical, and mental health challenges. Supporting individuals to be valued members of their local communities and optimizing their emotional, social, occupational, and learning experiences through community programs. This role will also involve providing 1:1 support to individuals with social activities and working with groups in the community.
Minimum Education & Skills Required
1. Minimum QQI Level 5 in Health and Social Care or equivalent is desirable.
2. Full Clean Driving Licence and access to a car for business purposes is required.
3. Experience in supporting individuals with intellectual, physical, and mental health challenges.
4. Experience in supporting community independence.
5. Experience in the provision of personal care support.
6. Knowledge and experience of local community resources is highly desirable.
Job Duties & Responsibilities
1. To plan, develop, implement, monitor, and review appropriate educational, recreational, and occupational programs within the community, ensuring best practices in line with up-to-date research regarding Quality of Life improvements.
2. To liaise with families, the multidisciplinary team, and key community-based personnel in pursuit of accessible community activities for service users.
3. To document information, update records, and complete necessary paperwork for communication and accountability purposes.
4. To ensure all interactions with service users are approached with dignity, respect, equality, and incorporate choice.

1. A flexible shift system will operate.
2. Lone Working.
3. A full, clean driving license with a minimum of 2 years’ experience is required.
4. Access to a car for business purposes.

Salary
Starting from point 1 €29,374.7 - point 5 €33,489.25 (pro rata), depending on experience.
