Duration: 2-year Contract
RequirementsAre currently employed in the public sector health service in Ireland at Consultant / Clinical Director or Specialist level with a minimum of 5 years' experience as a Consultant in an Acute or Community setting who are working within the specialty of Diabetes Diseases ANDRegistered with the Medical Council of Ireland on the specialist division of the register ANDPossess the requisite clinical, leadership, knowledge and the ability for the proper discharge of the duties of the officeRole OverviewThe effective and efficient management of people with Diabetes using an integrated approach for prevention, early detection, slowing disease progression and providing optimal treatment for quality and quantity of life. In line with Sláintecare, care should be delivered at the lowest appropriate level of complexity.
The National Clinical Programme for Diabetes should be the primary reference point for providing cross service Diabetes solutions in relation to new models/pathways of care based on local, regional and national population need, identified priorities and workforce projections.
Program DetailsThe NCP Diabetes is under the remit of the NCAGL Chronic Disease and works closely with other NCPs as part of the Integrated Care Programme for the Prevention & Management of Chronic Disease.
A significant programme of reform is underway in Services for Chronic Disease Management supported by the strategic direction set out under Sláintecare and the Enhanced Community Care business case.
Key Focus AreasStructural reform with Community Health Networks (CHNs) becoming the basic building blocks for community services.Creating Specialist Ambulatory Care Hubs for the management of Chronic Disease.Scaling Integrated Care for Chronic Disease through recruitment of acute specialist integrated care teams.The focus is on an end-to-end pathway, including Specialist Care Community Hubs that will prevent admissions to acute hospitals where it is safe and appropriate to do so. For patients requiring admission, the emphasis is on minimizing hospital stays and improving outcomes, with post-discharge support for people in the community.
In line with Sláintecare and the Department of Health's Capacity review, a shift in healthcare service provision is now required to focus on integrated, person-centered care, based as close to home as possible.
ConclusionThe successful candidate will retain all current terms and conditions (including remuneration and annual leave) on reassignment/secondment into the role. The standard working week associated with this post is 0.5 WTE per week, Monday to Friday. (Specific working days to be agreed on appointment). Clinical Practice will be facilitated.
