Job Title: Social Care Worker for Children's Residential Services
We are seeking a skilled and compassionate Social Care Worker to join our team in providing high-quality care and support to young people aged 11-17.
The successful candidate will work within a dynamic residential setting, supporting young people as they navigate their journey towards independence and adulthood.
Purpose of the Centre:
To provide short to medium-term transitional care for young people moving to and from Special Care Services.
Key Responsibilities:
* Provide person-centered care and support to young people, promoting their physical, emotional, and psychological well-being.
* Develop and maintain relationships with young people, families, and other professionals to ensure seamless transitions and effective communication.
* Participate in residential rosters, including evenings, weekends, and public holidays, to ensure 24/7 care and support.
* Work collaboratively with colleagues to create a safe, inclusive, and supportive environment.
* Contribute to the development and implementation of individualized care plans, ensuring that each young person receives tailored support.
* Maintain accurate records and reports, adhering to relevant policies and procedures.
Requirements:
* Degree in Applied Social Care (Level 7/8)
* Full flexibility to work a residential roster
* Experience in mainstream social care settings desirable
* Car ownership and full driving licence required
