Post Title:Clinical Nurse Manager 3 – Patient FlowPost Status:PermanentDepartmentPatient Flow DepartmentLocation:Beaumont HospitalReports to:Deputy Head of Patient Flow DepartmentSalary:Appointment will be made on Clinical Nurse Manager 3 Grade (€68,652 - €77,530) at a point in line with Government pay policy.Hours of work:Full time 37.5 hours per weekClosing Date:Thursday 26th December 2024 @ 12noon Position Summary: Patient Flow could be described as the management of all admissions, transfers and discharges by a hospital in a framework that integrates and coordinates all processes related to these activities. Discharge Planning is the main focus and core business of the Patient Flow department, which is fundamental to create essential capacity across the hospital in order to manage Patient Flow through the hospital effectively. The Patient Flow Department is a corporate department, which interfaces with and works effectively with Hospital Managers, Hospital Consultants, Community Services, Senior Nursing and other Senior Clinical Staff. Patient Flow is also responsible for monitoring and assessing the availability of beds to accommodate all admissions ensuring a real-time safe flow status at all times. The Patient Flow CNM3 will support the Head of the Patient Flow department in administration of the department, deputizing in her absence. In addition to managing real-time operational Patient Flow management and management of the department, the Patient Flow CNM3 will be assigned to lead on various projects focusing on effective Discharge Planning/Patient Flow management, liaising with senior stakeholders within the hospital and with community/external stakeholders. The Patient Flow CNM3 will manage and lead on the management of complex discharges.
Responsibilities
Principal Duties and Responsibilities:Working with the Head of Patient Flow the priorities are:• Ensuring adherence to building on the Visual Hospital (Patient Flow System) process and compliance with its associated procedures.• Leading and proactively managing/supporting staff in the discharge/flow of all patients in the hospital• Ensuring that effective policies (national and local), SOPs, PPPGs are in place and managed to support efficient admission and discharge of all patients• Maximising/promoting appropriate bed utilisation• Integration of unscheduled and scheduled care requirements• Reviewing trends in the patient activity i.e. length of stay, complex discharges compliance, admissions, transfers, discharges• Analysing, monitoring and forecasting trends of admission and discharges in the acute sector• Direct and active participation in service planning, implementation and evaluation and in overall resource management of the hospital, including continuous quality audit and performance measures specific to Patient Flow.• Ensuring the application of best practice including the development of action plans to continuously improve the effectiveness of the Patient Flow function in both the acute and non-acute sector.• He/she will have the authority to work directly with Directorate teams, Nursing management, Hospital management and Community/External stakeholders. He/She will be accountable for ensuring that the decision making process is effective and that remedial action is taken where necessary.• Responsible for the education/training of all staff in Patient flow management/discharge coordination. The Patient Flow CNM3 will support the Head of Patient Flow in:• Operational Management• Service Planning• Performance Management• Strategic Planning• HR Management and Organisational Development• Quality Assurance and Risk Management A. Operational ManagementI. Responsible for leading/supporting and managing the team in Patient Flow in the operations of Patient Flow ensuring the efficient and effective day to day running of the department. Responsible for matching resources to need with flexibility to respond to workload pressures.II. Develop and deliver effective operational plans linked to the hospital strategy and organisational development programme, ensuring that organisational and national targets are met.III. Implementing admission, discharge and transfer protocols, guidance and policies on the management of emergency workload and the timely placement of patients in the most appropriate environment in the acute or non-acute sector.IV. Constant monitoring and reporting of capacity KPI’s to senior management and circulation on a regular basis reports for key clinical decision makers on resource utilisation within the acute sector.V. Ensure that effective strategies/pathways are in place for the management of complex discharges in line with the Complex Discharge policy (2018)VI. Proactively develop policies/departmental strategies to support efficient discharge planning.VII. The post holder will be required to develop effective communications arrangements with colleagues in primary care and other hospitals/facilities.VIII. Manage the operational implementation of new service initiatives, including liaising with stakeholders, developing operational policies and guidelines as required. B. Service PlanningI. To be responsible for the co-ordination of the annual business plan for the Patient Flow Department, which contributes to the Hospital’s service plan with the HSE and in the future any groups or networks which the hospital is part of.II. Liaise with senior management to develop service plans and report frequently on progress of plansIII. Implement effective controls to monitor, evaluate and report the standards of delivery within the area of Patient FlowIV. Create a culture of accountability, openness to change, respect for the individual and teamworkV. To implement proven business management technologies within the department to strengthen performance and enhance its reputation internally within the hospital and externally with the public and the HSE.VII. To lead projects which improve the quality of services provided to patients evidenced by improved performance against national and international benchmarks.VIII. Providing required reports to outside agencies. C. Performance Management.I. To monitor, control and report on department activity working with the Head of the Patient Flow department.II. To be accountable for the effective use of resources, alerting the Head of Patient Flow in respect of imminent matters which may impinge on effective running of the department.III. Work collaboratively with the Head of Patient Flow and the Directorates to lead initiatives in strategic planning, development and redesign of services. D. HR Management and Organisational Developmenti. Lead and manage all staff within the Patient Flow Department, complying with hospital HR policies and procedures.ii. Actively promote and develop a positive working environment, ensuring a highly focused/skills/educated/motivated team.iii. Maximise the contribution of these staff by developing systems for performance appraisal, personal development, individual and organisational development and continued skill mix review.iv. Ensure that the responsibilities and objectives for direct reports are clearly defined and understood – job descriptions and person specifications up-to- date.v. In conjunction with the Head/deputy head of the department, regularly reviewing staff complements and skill mix across the department to ensure that staffing is appropriate for current and projected clinical and operational demands.vi. Be responsible for managing sickness and absence in accordance with hospital policy.vii. Act as a point of contact for staff on human resource issues in accordance with HR policies and procedures.viii. Be responsible for facilitating all staff to continue their education and personal development to support improved organisational performance.ix. Ensure the involvement of all staff through open and direct communication to sustain a high level of commitment and flexibility from the department workforce through implementing the hospital Communication Strategy and in particular the team briefings policy.x. Promote equal opportunity and adherence to best practice employment and approaches to managing and implementing change. E. Quality Assurance and Risk Managementi. Proactively lead and contribute to the development/implementation of best practice service modernisation and improvement plans and strategies within the Department.ii. Encourage a culture of continuous improvement and setting/supporting achievement of the highest possible standards with maximum efficiency.iii. To work with the wider hospital, stakeholders and colleagues to develop clinical governance systems within the department which ensure that all relevant HIQA and international standards are met.iv. To ensure that the department has effective procedures in place for risk assessment and risk management.v. To co-ordinate the Departments participation in local risk management initiatives to monitor clinical and non-clinical compliance with Health & Safety, Infection Control, Occupational Health, Fire Safety, environmental and organisational risks.vi. To co-ordinate the processing of adverse incidents within the department ensuring that they are investigated promptly, respecting confidentiality, instigating preventative action where necessary to fulfil IQS policies and procedures. Statutory Registration, Professional Qualifications, Experience, etc(a) Eligible applicants will be those who on the closing date for the competition:(i) Are registered in the relevant division of the Register of Nurses & Midwivesmaintained by the Nursing and Midwifery Board of Ireland (NMBI)(BordAltranais agus Cnáimhseachais na hÉireann) or entitled to be so registered.(ii) Have at least 5 years post registration full time (or an aggregate of 5 years postregistration full time) experience of which 2 years (or an aggregate of 2 years postregistration full time experience) must be in the speciality or related area.(iii) Have the clinical, managerial and administrative capacity to properly dischargethe functions of the role.(iv) Candidates must demonstrate evidence of continuous professional development.(b) Candidates must possess the requisite knowledge and ability including a high standard ofsuitability and clinical, managerial and administrative capacity to properly discharge thefunctions of the role.2. Annual registration(i) On appointment, practitioners must maintain live annual registration on the relevantdivision of the Register of Nurses and Midwives maintained by the Nursing andMidwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann).(ii) Confirm annual registration with NMBI to the HSE by way of the annual PatientSafety Assurance Certificate (PSAC).3.
