CNM II Patient Flow / Home Care Package Coordinator

PURPOSE OF THE ROLE
* The CNM2 will be responsible for the management of Home Care Packages for patients of Tallaght University Hospital.
* Have a responsibility in managing the home care package application process with patients and their relevant person.
QUALIFICATIONS
Each candidate must on the latest date for receiving completed application forms for the office -:
* Be registered in the General Division of the active Register held by Nursing and Midwifery Board of Ireland or be entitled to be so registered.
* Have completed a degree course to level 7 or higher in the NFQ framework
* Have 5 years post registration experience and a minimum of 2 years in the specialty or related area.
* Have the clinical, managerial, and administrative capacity to properly discharge the functions of the role.
* Have excellent communication and interpersonal skills.
* Demonstrate evidence of continuing learning and professional development
Desirable Criteria
* Evidence of management experience in an acute hospital setting
* Experience in discharge planning with knowledge of Home Care Package application process
* Training in SATIS system
2. Age Restrictions In Relation To Applications
Age restriction shall only apply to a candidate where he/she is not classified as a new entrant (within the meaning of the Public Service Superannuation (Miscellaneous Provisions) Act, 2004). A candidate who is not classified as a new entrant must be under 65 years of age on the first day of the month in which the latest date for receiving completed applications for the office occurs.
3. Health
A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health as would indicate a reasonable prospect of ability to render regular and efficient service.

5. Garda Vetting
Garda Vetting is sought for all employees and prospective employees of Tallaght University Hospital. Given the specialised nature of the services provided, your appointment will be subject to satisfactory Garda Vetting and re-vetting in circumstances where the Hospital deems it appropriate.
6. Mandated and Designated Persons under Children First Act 2015
Schedule 2 of the Children First Act 2015 specifies the classes of persons defined as Mandated Persons for the purposes of the Act. Mandated persons are required to report any knowledge, belief or reasonable suspicion that a child has been harmed, is being harmed, or is at risk of being harmed.
PARTICULARS OF OFFICE
1. The appointment is permanent, full time, and pensionable.
2. Remuneration:
Remuneration is in accordance with the salary scale approved by the Department of Health & Children. Current remuneration with effect from 1st June 2024:
€59,070 by 11 increments to €74,643 incl. 1 LSI
3. Pension arrangements and retirement age
There are three superannuation schemes currently in operation for staff in Tallaght University Hospital:
(a) Voluntary Hospital Superannuation Scheme (Non-New Entrant)
(b) Voluntary Hospital Superannuation Scheme (New Entrant)
(c) Single Public Service Pension Scheme
4. Duties:
The CNM2 will perform such duties as are outlined in this job description.
5. Hours of work:
Normal working hours are 37.5 worked over 5 days. Your contracted hours of work are liable to change between the hours of 8 am to 8 pm over 7 days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement.
6. Probation:
The successful candidate will be appointed initially for a probationary period of 6 months.
7. Annual Leave:
Annual leave entitlement is 25 - 28 working days, depending on length of service, plus 10 Bank Holidays per annum as they occur.
8. Sick Leave:
Payment of salary during illness will be in accordance with arrangements as approved from time to time by the Department of Health and Children.
9. Termination of Office:
The employment may be terminated at any time by two months’ notice on either side except where circumstances are dictated by the Minimum Notice and Terms of Employment Act 1973/91.

The person chosen will:
* Maintain throughout the Hospital an awareness of the primacy of the patient in relation to all Hospital activities.
* Ensure a high standard of care delivered safely in accordance with the hospital policies and procedures.
* Build good relationships with key stakeholders in the patient’s journey through hospital.
* Demonstrate knowledge of the Hospital/Departmental emergency plans.
* Ensure the provision of a high standard of care to the patient and families is consistent with the mission, vision, values and strategic plan of the Hospital.
* Demonstrate behaviour consistent with the Values of the Hospital.
* Commitment to continuous professional development including completion of relevant internal training programmes available through our Centre for Learning & Development Prospectus.
* Reporting relationship will be to the CNM 3 Patient Flow/Unscheduled Care Manager.
* Be professionally accountable to the Director of Nursing.
SPECIFIC ACCOUNTABILITY
Managing the Service – Quality and Safety of Service /Delivery of results (level 3)
Home Care Package Coordination
* Take a lead role in the management of Home Care Packages for patients of Tallaght University Hospital.
* Monitor applications from an early stage, track their progress, and liaise with Multidisciplinary team and SAT coordinators.
* Attend regular onsite meetings with discharge planners to assist in the prioritisation of funding for patients that are medically fit for discharge.
* Set targets with an aim to reduce patient’s length of stay.
* Gather relevant data and track delays in the HCP process.
* Ensure timely processing of applications and identify problems at an earlier stage.
* Update ward managers, Visual Hospital electronic system and planned patient pathway boards with discharge plan and Predicted date of discharge.
* Keep up to date with developments in area of HCP’s both nationally and locally.
Discharge Planning
* Assist the Discharge Planning Team and Bed Management with the effective and efficient use of hospital beds in accordance with hospital policy.
* Communicate with relevant members of the Multi-Disciplinary Teams on a daily basis to progress with patients discharge.
* Attend the daily operational bed management meetings.
Service Quality
* Support the Bed Manager and clinical teams in placing the right patients in the right place at the right time.
* Maintain a record of clinically relevant data aligned to national KPI’s as directed & advised by TUH & HSE.
* Participate in Quality Improvements Initiatives/Programmes.
