Job Summary:
This role supports individuals with intellectual, physical and mental health challenges in being valued members of their local communities.
Key Responsibilities
* Develop and implement individualized programs to optimize emotional, social, occupational and learning experiences for service users.
* Provide 1:1 support for social activities and work with groups in the community.
* Liaise with families, multidisciplinary teams and key community personnel to ensure accessible community activities for service users.
* Document information, update records and complete necessary paperwork for communication and accountability purposes.
* Maintain the privacy and confidentiality of service users at all times.
Requirements
* Minimum QQI Level 5 in Health and Social Care or equivalent is desirable.
* Willingness to participate in Rehab Care training programme and relevant external training opportunities as they arise.
* A full, clean driving license with a minimum of one year's experience is required.
* Access to a car for business purposes insured in your name.
