**Senior Pharmacist - Waterford Role Overview:**
We are seeking a Senior Pharmacist to provide effective, safe, and efficient pharmaceutical care to all patients. This is a full-time, permanent position.
Primary Duties and Responsibilities:
* Ensure patient safety and care is always the top priority.
* Understand, promote, and model the hospital's core values.
* Deliver and develop clinical pharmacy services, including prescription reviews, patient education, and clinical intervention documentation.
* Assist in managing the dispensary, participating in and supervising its operation, including procurement, storage, supply, and dispensing of pharmaceuticals and related items.
* Maintain best pharmaceutical practice, adhering to legal and ethical requirements, and best practice standards for pharmaceutical services.
* Ensure accurate records of procurement, storage, compounding, quality control, dispensing, distribution, and destruction of medicinal and pharmaceutical products, following best practice standards and applicable legislation.
* Achieve competence in using and administering the Oncology/Haematology clinical information system (ARIA).
* Become fully conversant with best practices and relevant competency frameworks for delivering Oncology/Haematology Pharmacy services.
* Support the Chief Pharmacist in managing new medication entry onto the hospital formulary, providing expert advice and education as needed.
* Develop and maintain Standard Operating Procedures (SOPs) for Pharmacy services, ensuring updates and implementation follow pharmacy law and changing practices locally, nationally, and internationally.
* Contribute to medicines information and advice on drug availability, use, and safe administration.
* Participate in developing medical information resources for clinical colleagues and patients.
* Collaborate with Pharmacy and Multidisciplinary Team colleagues to optimize Pharmacy and Medication Management workflows.
* Identify and address medication management system deficiencies.
* Report incidents/near misses to support risk management and quality improvement initiatives.
Qualifications & Experience:
* Registration with the Pharmaceutical Society of Ireland or entitlement to registration.
* Possess specialized knowledge and ability for the proper discharge of duties.
* Evidence of ongoing continuing professional development and role-specific learning.

Desirable Criteria:
* Formal management and leadership training or equivalent experience.
* At least 3 years of post-registration hospital experience.
* Broad clinical pharmacy experience and experience in line management.
* Experience in working within an aseptic compounding unit and/or haematology/oncology unit(s).
* Good clinical practice (GCP) experience.
* Further qualification, such as a post-graduate diploma or MSc in clinical pharmacy.
